Ran across this today. Case head separated and left half the shell in the chamber. Bottom half ejected and the bolt fed another cartridge into the broken shell. Bolt carrier had to be pried back with a screwdriver and ejected both the broken shell and cartridge.

SHooter next to me was in the rapid fire stage of the course.
Case split in half after extraction. One half of the case was stuck in the chamber and the next cartridge was chambered into the remaining case, hence the case over a case.

something similar happened to dmas once there as well. The neck came off a previous round and the next got shoved in. Put the rifle out of commission until he could get the small ring, which was the neck, out of the chamber.
At least the rifle didn't chamber and fire like that! :)
