I just printed out the affidavit, filled it manually, then scanned and signed it. PDF editors make it easier, but I've done it with a pen and phone scan before too.

I just printed out all the forms, filled everything by hand, and mailed it in. It took a while, but it went through fine without needing to do anything online.
