Medical Information Waiver Question (Read 1723 times)

Medical Information Waiver Question

Aloha,

Am I still able to apply for a handgun permit if I don't have a doctor?  I have Quest but haven't had to see a doctor since I was a minor, not sure if that doctor is even still in practice..

No problem.

Are you using the Quest integration with Kaiser or HMSA?  If so, you'll need to fill out an additional permission form for your healthcare provider.

Just put "none" for the doctor of PCP name.

as long as it's true
this is a state and federal form, so it is against the law to lie on them
and no one here does anything illegal...

I’m using Quest with HMSA; I didn’t know about the additional form, thank you for letting me know.

For the medical information form, where it asks for your doctor or health care provider, put the words “No Local Doctor”, and sign and date.

This was the information I was given when my doctor responded that she hadn't seen me in 3 years.
