Came off a Bushmaster Upper.  Not a quality issue.

Forging by Brass Aluminum Works (Square)

It was very bizzare. First time I seen this.

Guess how it happened!

Someone tried to remove the barrel without properly locking the receiver down?

but speed said he's never seen it before

Looks like the buffer struck it.  That would mean the upper and lower weren't being held together.

My guess would be running the thing without a buffer. But you would think if that were the case it would be pretty obvious something is wrong when you go to chamber a round and the bolt doesn't go forward on its own.

12 year old girl shooting.  somehow she was pressing down the fwd assist. it broke off the paw and lodged itself between the wall and bolt carrier!  The bolt was jammed halfway back......Lower was fine.
