Yes, it is real and yes I bought one.
The back story, from what I have gathered, is that 5.11 made this as an april fools joke a few years back. Well when they returned to work there were thousands of orders to their surprise. So now they release it every year.
So here is my basic review. Disclaimer, I have never worn a kilt before so I cannot compare it to anything.
It is made of standard ripstop style fabric so that is what it feels like. It closes with 4 snaps on the left and 4 on the right. Takes a little while to snap up but is very quick to remove. There is a 9th hidden snap half way down which keeps the the top flap from opening too much in case a nice breeze comes from just the right angle. There are two regular pocket which are very roomy. You then have two removable cargo pockets each with magazine compartments.
The fit is nice. You can feel it when even a light breeze comes through which is nice because it cools off your junk. As you squat, run, jump, etc it doesn't hinder you at all. I bought a pair of boxer briefs in dry-fit style fabric and this is important otherwise your thighs may rub. I wore it once with regular briefs and walked about a mile without any problem but the boxer briefs are definitely preferable. I sort of bought it as a joke but have been wearing it every weekend.
You will get a few odd views from people. So far a few people asked me about it, including two ladies!
