My lucky day. Around 8.15, walked right up to window to register. One guy filling forms, one waiting for permit. :)

thanks for the info.
did you go recently, bc now the form is different.
before we would fill out our Dr's name, now i notice we just fill in our Medical record number.
which leads me to belive it will not reach my doctor, but maybe just a receiving clerk.
but if you went recently, and this is how it goes, then thats fine with me.

Two and 1/2 weeks ago.  Form was filled out. Had the Kaiser Psy. address. I don't recall putting my med record no.

stangzilla,
I am a Kaiser member.  When I applied there was a printed form with the Kaiser info which I had to sign.  Waited the 14 days and picked up my handgun permit.

Got there at 8:40. Applying for handgun permit. Two people ahead of me: 1 person filling out forms and 1 person picking up gun. Guy after me, from out of state,  had 17 guns to register. Lucky, I was in front of him.  I finished at 9:10. :)

How can you be a notary to your own signature? The notary is suppose to be someone with authority to witness the signature other than the person signing the document.
