I spoke to the firearms department folk down at the HONOLULU Police Department on 801 S Beretania St in Honolulu. They stated the following:
 1 There are NO forms online they are at  the records/firearms office to be filled out.
 2 bring the registration paper and hand gun on file that will be used so they can get caliber serial number off it.
 3. Bring 2 passport photos
 

Once all these items are brought I am to fill out the forms there. If a person has never been there before then it requires the $16.50 background check fee. 
The phone number is 808 723 3190.

I will update as I go.

Thanks for the update. Yes, please keep us informed of the details. Any possibility that you could take a camera capable of taking a decent quality photo of the documents prior to you filling them out? Only, of course, after asking if that is okay/legal. If they say no, you might ask them to show you the statute prohibiting it. Or not.  :shaka:

The form is at the window only and not allowed to leave the room.  Let us know what happens if you try to take a picture of it.  I don't think they want to let it out because legal will start to put holes in it and the process.  But it should be a publicly available form to begin with!

If you are stopped from taking a photo of it, please let us know.  I was personally threatened with arrest for trying to leave with the documents.

There is nothing prohibiting it.  I always let them know that I am recording them (I'm sure they love me).

Hey Chris, have you ever thought about making a FOIA request for those documents? Just curious how one might force their hand in terms of releasing documents applicable to the process for applying for a CCW....

ACLU backed us and compelled the entities that refused to produce the documents.  It took a few letters to Kauai, but they finally gave it up.   Honolulu was the same way.  I tried to get some more stuff, but we are precluded due to litigation (they have an exemption for that); we shouldn't be excluded, but that's their excuse and I didn't have the legal resources to challenge it.   Other individuals could also submit the form.

Good updates.  From what I have read before, it seems that when applying for a carry permit here, it only applies to one specific firearm.  From what I've read from people in other states it seems they can switch their carry gun at will?  ("Thinking of switching to a full size for winter carry, compact during the warm months etc.")  So if we wanted to switch carry guns we would need a whole new permit, unlike other states?

I cannot imagine the form being much different from other states ccw applications. If you cannot copy or duplicate what you sign that is pretty strange. But I will ask for sure.

From a certain perspective (the commonly understood meaning of the words "the right of the people to keep and bear arms shall not be infringed"), most every Hawaii statute infringing the right to keep and bear arms ("permit to acquire", waiting period, "registration", magazine size limit, ban on "assault pistols", ban on the "bearing" of arms, can't stop to pee or buy gas if you have a firearm in between "legal" destinations, etc. etc. etc.) are ALL "pretty strange".

I fully agree boss.I lived in the Gunshine state for 13 years and coming our here suprised me on the various point you bring up. well said good brother.
