Recently acquired a Colt Combat Commander 1911.
Took it out to the range a couple weeks ago.
First couple of mags were good, then failure to extract on second to last/last round.
Tried different mags and ammo with same results.
The extractor was pulled and inspected.(really fouled up)
Didn't notice anything real obvious.
Anybody willing to take a look and give advice?

Not the first owner.
According to Colt it was manufactured in 1981.
Previous owner did not know history.

How many mags did you try?  Were they all the same brand?

Percentage-wise, how often was it the last round versus the next-to-last round?

I know a little bit about 45 autos.
Let's get together. If we meet, I need to see all the ammo and magazines and spent shells too if you got them.
If using reloads, i need the load data too.

Tried 5 different mags, all GI style. One stainless, the others steel Chip McCormick.
When issues first started it was last round, then consistently second to last round.

Various ammo was used from Aguila, RWS ,Federal, to what ever loose rounds I could find.
Did not keep spent shells.
Nope, not reloads.

When it failed to extract on the next-to-last round, I assume the last round was stuck there, pressing against the spent round, the slide resting on it.  Yes?

What about when the last round failed to extract, did the slide lock back?

This occurs multiple times? The case is still stuck in the chamber?
If such is the case, there is not a very quick method of clearing that.

Method 1 is to drop the mag and let the slide slam into battery and pray the claw will grab the rim and extract the case.
You could try it once, but if the problem repeats you could damage the extractor.

Method 2 is to drop the mag and pry the shell out with a knife or screwdriver

Method 3 would be to push the case out from the muzzle end with a rod.

Had multiple malfunctions.
First time was drop the mag and turn over on its side-cleared.
Second time was drop the mag, rack the side a couple times and cleared.
After that, it was second to last round issues.

There's only a few reasons for malfunctions like yours.

Got it figured out.
Another adjusted extractor and it cycles snap caps.
No range time.... yet
