Not a real doctor.

The AP style guide says to not use the title "Dr" for a non-medical doctor.  If you do, then you should also add the type of doctor for clarification, and only if the title is relevant in the context of the writing.  In Jill's case, only use "Dr." if the context is in the field of education.

This is obviously for purposed of written communications like news reports.  For other purposes, like official functions, speech introductions, and so on, it's up to the individual based on the situation.  I would not personally introduce her as Dr. Jill Biden to a room full of doctors unless it's a discussion on education improvements for medical schools.

I worked with so many PhD's,  I lost count.
Most had PhDs in Astro Physics or Mathematics.
Most were all good friends.  Free education, for me,
when you work with these people you get tested everyday.
I only remember one that insisted on being called "Doctor"
He didn't last long in the group.
I think he was JPL.
You have probably seen him on TV.
He is gone.

I'm on the fence as far as opinions on the FLOTUS.

But upon learning she is 69 I am impressed at how she looks a lot younger for her age.  And she also looks fit and is good looking.

Her husband also looks fit physically but I'm not sure mentally and his face reminds me of a leatherback turtle.  But I digress......
