This is obviously for purposed of written communications like news reports.  For other purposes, like official functions, speech introductions, and so on, it's up to the individual based on the situation.  I would not personally introduce her as Dr. Jill Biden to a room full of doctors unless it's a discussion on education improvements for medical schools.

Looked up her age, she is 69. She isn't my type but I gotta say, pretty good looking for 69.
