how to notify hpd of transfer/sale

tried search but 2015 keeps popping up.  selling a firearm, how to notify hpd of transfer/sale?  I usually meet at hpd etc but now with appointments being so far out how do I do it? certified mail? what address?

Name
Addy
Make/ model/ Type of firearm ( semi auto or shotty or bolt action) / barrel length/ caliber/
Serial number
Permit number (added insurance so no mixups or just incase you wrote something wrong)

Buyers info:
Name
Addy??? (Not sure bout that)

Blah blah call me at xxxxxxxxxx, if you have any questions, etc.

Found it. Bout 5 or 6 posts down, read stang's post. He sourced it from hpd themselves.

Thanks Aaron for the link, Stanz prep the info on the Jan 4th post and if it helps a fillable pdf was attached.  That way you can save it and just update for the next sale.

Sounds bout right as a header. Just need to put "honolulu police department" above records and id div. , firearms section.

Yeah just put todays date in the letter. Cause i think its 2 or 3 days standard for islandwide mail to be delivered. Hpd knows that.  So if you sell it today and lose possession of it today. Write todays date.. also state.. " i am transferring ownership ("of said weapon") to ....(buyers name) ..." i know they want that.
