Using the Online PTA forms - are they current?

Are the online forms current?

One time a while back, I printed and filled out the forms but hpd only used a few of the forms and I had to use the forms at hpd

All the forms are current. The firearms information form is not used for the permit side; only for the registration side. Signing is fine, just make sure you date the forms for the day you’re applying. For the medical info one (doctor form), the witness is the HPD person at the window, not a family member, etc. Hope that helps.

It does not hurt to fill out a Firearms Information Form (HPD084R05) for a handgun as it contains information you will need to transcribe to the official multipart Application for Permit to Acquire Firearms (this has a G on top with a unique 6 digit code). I found the HPD clerk uses the HPD084R05 form you get online (or dealer weapon description form) to populate the seller info onto the Application for Permit to Acquire Firearms. When countersigned and approved (14 days later), the Application for Permit to Acquire Firearms becomes your official permit to acquire. Knowing the timetable for the entire application process is of the highest importance. We may be up to application form number 300000 by now. Whether this means that many forms have been submitted or some fraction thereof, this is quite a large number. A bureaucrat's dream or nightmare?
