Just today there is a new PTA form to fill out
It's the same questions but on 8 pages instead of 3
It's not available online
So expect delays

One would think they would have the form available online pretty quickly. 

Maybe they are trolling for another lawsuit?

I got there around 1145, went inside and grabbed the forms and went outside in line and filled em out.  By the time I got in, the lady said there’s a new form.  Me and another braddah was like, “We just got these from the tray…”.  She said they just changed it!  😂  “There’s 8 pages now.”

So I had to redo the initials section, and she just restarted the front page.  What a fiasco…

the "AG Revised 03/2023" version was 10 pages, currently still posted on-line.  They need to huddle in the back and get their stories straight.   :wacko:

I just checked HPD's website.

Is this the new PTA form  that is being used now or is it different?  The whole thing is like 12 pages, but supposedly you only need the first 8 pages, is that right?

yes, that's the one
but when I went HPD last week, they only had up to page 8
