Deformed brass?

What might cause this unusual shape?
7.62x39 factory ammo.
It's not mine. I just found it.
(I have my theory but want to hear other opinions)

The affected rifle was built by a supposedly well known mainland gun smith.
The owner of the rifle was told by several very experienced people he has a serious problem.
The owner appeared to not fully understand nor appreciate the concern of others. .
If any of you know the owner of the rifle, please tell him to contact the gunsmith.
 He should at least text him a pic of a fired case asap.

Improper cut chamber.
Barrel manufacters use boring bar to cut chamber for mass produced barrels.
1 that fell thru the cracks. Smith most likely test fired but did not inspect fired case.
